Please find below forms related to new patients:
Your First Appointment
1. Please bring your Medicare card with you and all the necessary documents/GP letters, audiograms/hearing test results, needed for your appointment with Dr Jardeleza. Please also complete the New Patient Registration Form (link located below).
2. If any CT scan, MRI, xray or imaging has been done that relates to your ENT visit, please inform us on your information sheet, as well as where the scans were done (which imaging company did the scan).
3. If you need a hearing test before your clinic visit, my staff will contact you or your GP to have this arranged prior to your visit. This may save you time especially if you live far from Wagga Wagga. Please bring the results with you on the appointment day. Alternatively you can let my reception know where you had the hearing test done and we can find the results for you.
4. If you need a hearing test for your visit and this has not occurred nor previously arranged, we will arrange one for you on the same day as your clinic appointment. This will be with Clayden Hearing, located in the Docker Medical building, about 5 minutes from the consulting rooms. The reception staff will arrange this for you and will advise you via a phone call or text 2 days prior, to go to Clayden Hearing first before coming to your clinic appointment with Dr Jardeleza.
5. We are a new clinic practice, so unfortunately we cannot bulk bill.
Upcoming Practice Information
Tonsillectomy +/- adenoidectomy, Septoplasty, turbinoplasties/turbinate reductions, FESS/ sinus surgery
– Please inform Dr Jardeleza if you are on any blood thinners, so she can plan and liase with your GP/other specialist to stop these medications safely, prior to your surgery
– Please stop any medications/supplements with potential for blood thinning, at least 7 days prior to your surgery: Fish oil, Krill oil, Gingko biloba, herbal medications etc.
– Not stopping blood thinning medications at an appropriate timeframe before surgery can result in the delay, postponement or cancellation of your procedure.
– For other surgical procedures, Dr Jardeleza or her staff will let you know during your clinic visit, if these medication need to be ceased
Information For Patients: Aftercare
We would like to provide information and after care for our common surgeries. If there are any additional queries, please call the clinic:
1. Adenotonsillectomy or tonsillectomy– It is generally an overnight stay in hospital. After surgery, the time off school or work and rest at home is 2 weeks.
If you or your child live over 1 hour away from the Wagga Wagga Base Hospital, we heavily recommend to stay within the 1 hour vicinity of the WWBH for at least 10-12 days after surgery. This is due to the risk of bleeding which is about <= 5%. There are no other ENT surgeons who permanently reside in the Riverina region except in Wagga Wagga, hence it is best to stay within the 1 hour vicinity during the recovery period. We do not want any delay in you/your child’s care should a complication arise, which can be a great risk to you/your child.
There is a Ronald McDonald house close to the hospital where most of our patients living out of town, stay. Please contact them directly for accommodation.
2. Septoplasty, turbinoplasties, FESS/sinus surgery – it can be a day procedure or overnight stay, depending on your medical circumstances. Dr Jardeleza will let you know during your surgery discussion. You will be given a pamphlet for after care.
3. Grommets – this is a day surgery procedure. You will be given a pamphlet for aftercare.
